Thanks for taking a moment to register your household's information with our parish's office. Please fill out the form as completely as possible and then click submit.


We ask for the date and place of Baptism.  Please know that even if you were baptized in another faith tradition, If they use water for their baptisms, usually it still counts as being baptized.  The date of Baptism in another church can be offered.
