No payment is needed because of a generous grant from the Diocese of Springfield in Illinois.

I authorize the Diocese of Springfield in Illinois, Deaf Ministry Office to use pictures taken of my child during program events for presentations, marketing, advertising, promotion, and instruction of the church.

I also authorize Christine Lansaw, as the coordinator of Deaf Ministry for the Diocese of Springfield in Illinois, and/or Fr. Adam Zawadzki, as the Director of Deaf Ministry for the Diocese of Springfield in Illinois, to take my child on Church and Diocesan Sponsored field trips, Masses, or other Church affiliated events outside of school hours.
